If you need any vaccinations relating to foreign travel you need to print out and fill in the travel vaccinations questionnaire. Once you have filled in the questionnaire, return it to the surgery. The receptionist will then book you an appointment with the practice nurse. This appointment will need to be at least 2 weeks before you travel to allow time for the vaccinations to work.
Some travel vaccines may incur a charge. Not all travel vaccines are included in the services provided by the NHS.
It is important to return the questionnaire and make an appointment at least 8 weeks before you travel, as vaccinations may need to be ordered in.
